Eligibility:
a.10+2 pass preferably science with (Physics, Chemistry, Biology) & English with an aggregate of 40 % marks.
b.10+2 pass with Arts & English with an aggregate of 40% marks.
c. 10+2 with English having 40% of marks in vocational ANM course from a school recognized by the Indian Nursing Council.
d. Registered ANM with pass mark. e. The Minimum age should be 17 years. f. Should be medically fit to undergo the course.

How to apply for GNM Admission
1. To get an application form for the GNM course, candidates must apply to the
Principal, School of Nursing, along with an application fee of Rs.500/- by Demand
Draft/Cash drawn in favor of Baptist Christian Hospital payable at Tezpur. Applicants must give their full address in Block letters with pin code. Or bank transfer
can be made to School of Nursing, Baptist Christian Hospital, India Overseas
Bank, A/C No 129001000006065; IFSC IOBA0001290; Tezpur.

2. The form and prospectus is available online and application can be made online. The application fee as mentioned above can be done through a demand draft or bank transfer and the demand draft and online bank transfer slip can be sent to the address given below by registered post or courier. Please mention your application number that will be sent to you by email after submission of the form online.

4.The application form is to be completed by the applicant in her/ his own handwriting or online correctly. Full name of the candidate as seen in high school records should be given in the application form clearly.
5. All information given in the application form should be correct. Any false certificates/ information in the application would lead to disqualification. If the same is discovered during the course of training, the candidate will be liable to be terminated.
6. Completed application form along with necessary enclosures should be sent to the Principal so as to reach on or before 15th June’2019, 5pm

Documents to be submitted along with duly filled Application form. Duly

attested photocopy of the following.
a.Birth certificate.
b. Admit cards of class X & XII.
c. Marks sheet of class X & XII.
d. Pass Certificate of class X & XII
e. Caste certificate. – ST/SC/OBC/MOBC
f. Recent passport size photographs – 4 nos. in an envelope.
g. Income certificate.
