i) 2 Copies of recent passport size colour photograph duly self attested
ii) Birth Certificate/ HSLC Admit/ HSLC Board pass Certificate for age proof
iii) All pass Certificates from Boards/ Universities from HSLC Onwards
iv) Caste Certificate (where applicable)
v) Permanent residentship certificate/ Domicile Certificate (if any)
vi) Employment Exchange Registration Certificate
vii) EWS Certificate (Wherever applicable)
viii) Diploma in ICU Technology course from Govt. recognized institute
