1.Name of Post: Statistical Investigator (SI)
Total no of Posts: 1
Educational Qualification: Graduate in any stream/ Minimum 6 (six) month Diploma/ Certificate in computer application (Attach self attested valid supporting documents)
Age Limit: 18 years to 38 years. ( For age relaxation check notification)
Scale of Pay: Rs. 14,000/- to rs.60,500/- with Grand Pay 6,800/- PB-2

2.Name of Post: ANM
Total no of Posts: 4
Educational Qualification: ANM course passed from any recognized institution by Indian Nursing Council and must be registered under ” Assam Mid Wives & Health Visitor Council ” (Attach self attested valid supporting documents).
Age Limit: 18 years to 38 years. ( For age relaxation check notification)
Scale of Pay: Rs. 14,000/- to Rs.60,500/- with Grand Pay 6,200/- PB-2

3. Name of Post: Field Worker
Total no of Posts: 2
Educational Qualification: HSSLC Passed/ Minimum 6 (six) month Diploma/ Certificate in computer application (Attach self attested valid supporting documents)
Age Limit: 18 years to 38 years. ( For age relaxation check notification)
Scale of Pay: Rs. 14,000/- to Rs.60,500/- with Grand Pay 6,200/- PB-2

4. Name of Post: Computer
Total no of Posts: 2
Educational Qualification: HSSLC Passed/ Minimum 6 (six) month Diploma/ Certificate in computer application (Attach self attested valid supporting documents)

How to Apply:
The candidates must be a citizen of India
The applicant should clearly write “APPLICATION FOR THE POST OF” on the front cover of the envelope
i)The Applicant should submit the application form addressed to the office of the Addl. Chief Medical & Health Officer (FW), Rongkhelan, Diphu along with his/her complete postal address.

ii) The eligible candidates shall apply in standard form of application along with KAAK Money receipt of Rs. 10/- (ten) as application fee. The eligible candidate must submit 3 (three) Nos. of passport size photograph affixing one, self attested copy on the application form.

iii) Self attested Xerox copy of all relevant certificate/ Mark sheet/ Cast certificate etc. have to be submitted along with the application.
iv) The original documents/ certificates and other item specified in the intimation letter/ Admit card for interview are to be produced at the time of interview, failing which the candidate would not be allowed to appear in the interview/ Examination.

v) The candidates willing to apply for more than one post shall have to submit separate application form for each post.
