Documents on the day of the counseling
- Identification proof having name of both mother & father
- Surety Bond in 100 Rupees noon Judicial Stamp paper (content of the surety bond is uploaded in the NHM website)
- Passport size photographs with plain background (within last 6 months)
- Copies of IGNOU Blank Application form with ID Card – Candidate will be made to fill the application form during the counseling
- The Candidates are also Instructed to bring 1 xerox copy of the below mentioned documents
- Registration Certificate of last qualifying examination (BSC/POTS Basic BSC/GNM)
- Provisional/Final pass certificate of nursing
- In case of SC/ST Candidate, caste certificate issued by the competent authority

Venue of counseling: Assam Water Centre, Basistha, Guwahati
If any seat is left vacant during the course of the counseling then waiting list candidates will be called separately on a fresh date and time which will be notified later. All admission procedure is subject to the approvals of Indira Gandh National Open University (IGNOU)
